I/Q Follow-Up Tab |
Purpose |
The I/Q Follow-Up tab allows the user to record details regarding the client's isolation/quarantine episode. The user may record information about the client's isolation/quarantine location, precautions, the client's special needs, disease, and disease-related notes.
Entering or Editing I/Q Follow-Up Information |
To enter or edit I/Q follow-up information, follow these steps:
Click on the 'manage client' hyperlink from the Menu Panel on the left side of the Main screen. The Client Search screen displays.
Enter client search criteria for the client whose I/Q Follow-Up information you want to edit. Click the Find button.
If the system finds more than one client who matches the search criteria, the system displays a listing. Choose the one client you want to edit from the listing.
On the Edit Client screen, click the I/Q Follow-Up tab.
Enter new information or edit existing information for the fields on the I/Q Follow-Up tab. None of the information on this tab is required. For a description of each field, see the Information Provided section below.
If you are satisfied with all the information you have entered, click the Save button. Otherwise, click the Cancel button to have the system to restore the client's data to the way it was prior to your edits.

The following table describes the input fields on the I/Q Follow-Up tab on the Edit/Enter New Client screen:

Description |
Case Classification |
Category describing whether the client's isolation/quarantine case is confirmed, probable, suspect, lab only, exposed, or not ill. |
Location |
Type of location where the client is in isolation/quarantine. The choices are hospital, nursing home, home, community facility, and other. |
Total Persons at Location |
Number of people who are at the same location as the client in isolation/quarantine. |
IQ Address |
Address of the isolation/quarantine location. The user may specify street address, city, state, and zip code. |
Telephone |
Phone number of the isolation/quarantine location. |
Location Notes |
Free-text notes regarding the isolation/quarantine. |
Disease |
Disease which the client has. The choices are cholera, gastro-intestinal illness, influenza, pertussis, plague, respiratory illness, SARS, smallpox, viral hemorrhagic fever, and other. |
Disease Notes |
Free-text notes regarding the client's disease. |
Infection Control Precautions |
Manner of disease transmission. This field indicates what precautions people coming into contact with the client should take. The choices are contact, airborne, droplet, and eye protection. |
Special Needs |
Special needs, possibly non-medical related, that the client may have while in isolation/quarantine. The choices are special diet, medications, child care, pet care, disability, and other. |
Special Needs Notes |
Free-text notes describing the client's special needs in greater detail. |
General Notes |
Free-text general notes regarding the client's isolation/quarantine episode overall. |
Functionality |

When you click on the Add button, the system adds a selected item from the left-hand box to the right-hand box. All infection control precautions or special needs in the right-hand box are saved on the client's record. Make sure to click the Save button in the upper right-hand part of the screen to finalize the change. |

When you click on the Remove button, the system takes the selected item in the right-hand box and moves it to the left-hand box. This removes the selected item from the client's record. Make sure to click the Save button in the upper right-hand part of the screen to finalize the change. |
