Personal Information Header

 Purpose

The Personal Information Header comprises the top section of the Edit/Enter New Client screen. The Personal Information Header displays fields often used to distinguish among individuals when executing client searches. You may make edits to the fields in the Personal Information Header. Only SNS Developer, SNS System Manager and SNS Help Desk roles may edit an existing social security number.

 Required Fields

The following table describes the required fields in the Personal Information Header section of the Edit/Enter New Client screen. *Note*

Field Name

Description

Last Name

This is the client's last name. Assign or change a last name by typing the desired name into the Last Name text box.

First Name

This is the client's first name. Assign or change a first name by typing the desired name into the First Name text box.

Mother's Maiden Last

This is the client's mother's maiden last name. Assign or change the mother's maiden last name by typing the desired name into the Mother's Maiden Last text box.

Mother's First Name

This is the client's mother's first name. Assign or change the mother's first name by typing the desired name into the Mother's First Name text box.

Birth Date

This is the day on which the client was born. Assign or change the birth date by typing the desired date into the Birth Date text box using the format MM/DD/CCYY. Or, click on the calendar icon to set the date.

 Other Fields

The following table describes the non-required fields in the Personal Information Header section of the Edit/Enter New Client screen.

Field Name

Description

Middle Name

This is the client's middle name. Assign or change a middle name by typing the desired name or initial into the information field directly following 'Middle Name'.

SSN

This is the client's nine-digit social security number. Assign a social security number by typing in the nine-digit number into the three information fields directly following 'SSN'. Note that SSN is a unique field. Two distinct clients in the system are not allowed to have the same social security number.   *Note*

Gender

This is the client's gender. Assign or change the gender by clicking on one of the three radio buttons directly following 'Gender'. If a gender type is not selected, the system will default to 'Unknown'.

County

This is the county where the client currently resides. Assign or change the county of residence by selecting one of the counties from the combination box directly following 'County'.

Country of Birth

This is the country where the client was born.  Assign or change the country of birth by selecting one of the countries from the combination box directly following 'Country of Birth'.

Last Updated Date

This read-only field displays the last date that this client was entered or edited by a provider organization, as well as the name of the provider organization.

 Field Validation

The following table describes some of the validations the system performs on the data entered in the Personal Information Header section of the Edit/Enter New Client screen.

Field Name

Last Name

The system only accepts alpha characters, dashes, and apostrophes.

First Name

The system only accepts alpha characters, dashes, and apostrophes.

Middle Name

The system only accepts alpha characters, dashes, and apostrophes.

Mother's Maiden Last

The system only accepts alpha characters, dashes, and apostrophes.

Mother's First Name

The system only accepts alpha characters, dashes, and apostrophes.

SSN

The system only allows numeric characters that do not repeat (999-99-9999. 111-11-1111, etc) or are sequential (123-45-6789, 987-65-4321, etc) The system will not accept duplicate social security numbers. If you enter a SSN that is already in use by another client and save, the system will save the other edits for the client, but give you a validation warning that the duplicate SSN is not saved.

 Notes

In order for the system to create and maintain a record for a client, there is a minimum amount of information necessary that a user must provide. These select pieces of information are the Required Fields and are denoted in the registry by the blue information field labels as seen in the example below.

Mother's First Name

Once a social security number has been entered and saved, it will not be displayed. Instead, directly after 'SSN', the registry will display the social security number as '(On File)' or as a series of solid dots, depending on your user role. Therefore, exercise caution when entering a clients social security number. If it is entered incorrectly and a search is executed on the manage client/prophylaxis screen using SSN, the client will not be located.

If your user role is Help Desk, System Manager, or Developer, you may make corrections to the SSN.  If you have one of these roles, you will see a series of solid dots, which represents the existing SSN, and you may overwrite it with the new correct SSN.
