Client Reports Screen

 Purpose

The Client Reports screen allows the user to run one of the following three PDF reports: the Isolation/Quarantine Individual Follow-Up Report, the Voluntary Client Isolation Contract, or the Isolation Orders Report.

 Run an SNS Report for the Client

The user may choose to run one of three reports for a single client. These are the Isolation/Quarantine Individual Follow Up Report, the Voluntary Client Isolation Contract, and the Isolation Orders Report. To run one of these reports, follow these steps:

  1. Click on the 'sns manage client' hyperlink from the Menu Panel on the left side of the Main screen. The Client Search screen displays.

  2. Enter identifying information on the Client Search screen, and click the Find button. If several clients match the search criteria, the system displays a listing of possible matches. Select the client you want. The Edit Client Screen displays.

  3. Click the Reports button. The Client Reports screen displays.

  4. Select one of the following three reports by clicking on the hyperlink: Isolation/Quarantine Individual Follow-Up Report, Voluntary Client Isolation Contract, or Isolation Orders. The system displays the report in PDF format.

  5. You may use the print button on the PDF toolbar to print a hard copy of the report.

 View the Client Information Header

To view the client information header, follow these steps:

  1. Click on the 'sns manage client' hyperlink from the Menu Panel on the left side of the Main screen. The Client Search screen displays.

  2. Enter identifying information on the Client Search screen, and click the Find button. If several clients match the search criteria, the system displays a listing of possible matches. Select the client you want. The Edit Client Screen displays.

  3. Click the Reports button. The Client Reports screen displays.

  4. You may view the information in the client header, which displays at the top of the screen. The client's name, date of birth, gender, mother's maiden name, chart number, address, and symptoms all display.

 Information on the Isolation/Quarantine Individual Follow-Up Report

Here is a snapshot of the Isolation/Quarantine Individual Follow-Up Report, which provides client information that can be used for isolation and quarantine follow-up:

 

The following table describes the output fields on the screen:

Field Name

Description

ID Number

The VIIS ID for the client.

Name

The name of the client.

Home Address

The home address for the client. The home address is entered on the Contact(s) tab of the Edit/Enter New Client screen. The address of the primary contact/responsible person for the client displays on all SNS reports. The street address, city, state, and zip code all display.

City

The city where the client resides.

Home Telephone Number

The home telephone number of the client.

Date of Birth

The date of birth for the client.

Age

The age of the client in years. Decimal places may display.

Gender

The gender of the client.

Health Care Provider Name

The name of the Provider-PCP for the client. The user enters the Provider-PCP on the Client Information tab of the Edit/Enter New Client Screen.

(PCP) Address

The address of the PCP. The street address, city, state, and zip code all display.

(PCP) Telephone Number

The telephone number of the PCP.

Disease

The disease for the client. The user enters disease on the I/Q Follow-Up tab of the Edit/Enter New Client Screen.

Date of Onset/Exposure

The date when the client was initially exposed as entered on the I/Q Follow-Up tab of the Edit/Enter New Client Screen.

Source

The number and name for the event which causes the client to be in isolation and quarantine.

Case Classification

Indicator of whether the case of isolation and quarantine is confirmed, probable, suspect, lab only, exposed or not ill.

Type of Restriction

Indicator of whether this is an isolation or quarantine case (or neither).

Isolation/Quarantine Start Date

The date when the isolation/quarantine began.

Isolation/Quarantine End Date

The date when the isolation/quarantine ended.

Location of Isolation

A category describing the type of location at which the isolation/quarantine of the client is occuring. Hospital, nursing home, home, community facility, other may display in this field.

(Location) Address

The address of the location at which the isolation/quarantine of the client takes place. The street address, city, state, and zip code all display.

(Location) Phone

The phone number of the location at which the isolation/quarantine of the client takes place.

Free Text Notes for Location

Notes regarding the location at which the isolation/quarantine of the client takes place.

Infection Control Precautions

The type of precaution needed when coming into contact with the client in isolation/quarantine. Airborne, contact, droplet, eye protection are values which may display.

Names of Permitted Visitors

The names of individuals who may visit the client in isolation/quarantine. Only individuals on the Contact(s) tab of the client with the Permitted Visitor checkbox checked will display here.

Special Needs

Special needs the client may have as a result of being in isolation/quarantine. These include medications, pet care, disability, special diet, child care, other.

Special Needs Notes

Free text notes related to the client's special needs.

Emergency Contact Information

The first name, last name, and phone number of the primary contact as indicated on the Contact(s) tab of the Edit/Enter New Client Screen.

Total number of persons isolated/quarantined at same location

The number of persons isolated/quarantined at the same location. The user enters this number on the I/Q Follow-Up tab.

Comments

User-entered general notes regarding this client's isolation/quarantine episode.

 Information on the Voluntary Client Isolation Contract

The Voluntary Client Isolation Contract provides a legal document requesting voluntary isolation and highlighting the procedures that isolation will involve. Here is a snapshot of the contract:

 

Certain fields pertaining to the client's isolation and quarantine episode are embedded throughout the contract. The embedded fields display in bold. The table below describes the output fields on the Voluntary Client Isolation Contract:

Field Name

Description

Client Name, Date of Birth, Address

The client's name, date of birth and home address all show on the first line of the report. The street address, city, state and zip code all display.

Disease (Paragraph 1)

The disease which the client in isolation/quarantine has.

Location (Paragraph 1)

The location of the client in isolation/quarantine.

Disease, Symptoms, Disease Notes (Paragraph 2)

The disease, symptoms, and free text disease notes for the client in isolation/quarantine.

Isolation/Quarantine (Checkbox 1)

This indicates whether the client is in isolation or in quarantine.

Location (Checkbox 2)

The location where the client has been isolated or quarantined. The user enters the location on the I/Q Follow Up tab of the Edit/Enter New Client Screen.

Permitted Contacts (Checkbox 3)

The names of individuals who may come into contact with the client who is in isolation/quarantine.

 Information on the Isolation Orders Report

The Isolation Orders Report provides a legal document ordering isolation and highlighting the procedures that isolation will involve. Here is a snapshot of the Isolation Orders Report:

Fields associated to the client's isolation/quarantine episode are embedded in the paragraphs and text accompanying the checkboxes on the report. Embedded fields display in bold. The following table shows the output fields on the Isolation Orders Report.

Field Name

Description

Client Name, Date of Birth, Address

The client's name, date of birth and home address all show on the first line of the report (the greeting). The complete address displays, including street address, city, state, and zip code.

Health Officer (Paragraph 1)

The user who has printed out the orders.

Case Classification & Disease (Paragraph 1)

The case classification and disease which the client in isolation/quarantine has.

Location (Paragraph 1)

The location of the client in isolation/quarantine.

Case Classification & Disease, Symptoms, Disease Notes (Paragraph 2)

The case classification for the client's isolation/quarantine episode. Disease, symptoms, and free text disease notes for the client in isolation/quarantine.

Isolation/Quarantine (Checkbox 1)

This indicates whether the client is in isolation or quarantine.

Location (Checkbox 2)

The location where the client has been isolated or quarantine. The user enters the location on the I/Q Follow Up tab of the Edit/Enter New Client Screen.

Permitted Contacts (Checkbox 3)

The names of individuals who may come into contact with the client who is in isolation/quarantine.
