Accept Remote Client Screen

If this is the remote client, you can click on Accept to begin processing the client into VIIS.  NOTE: If the client does not have any immunizations, the Accept button will not display and in its place the message: The remote client returned does not have immunizations. This client cannot be accepted here.

When the Cancel button is clicked, the user will be taken back to the Remote Client Search screen. Fields that were used in the query will still contain data.

Client's Name 

Client's first name, middle name and last name.

Birth Date

Client's date of birth.

Street

Street address of the client.

City, ST ZIP

City, State and Zip Code for the client.

Mother's Maiden Name

Client's mother's maiden first name, middle name, and last name.

Responsible Person

Index number.

Relationship

Relationship to the client: Father, Mother, Self, etc.

Name

First name, middle name and last name of the responsible person.

Street

Street address of the responsible person.

City, ST ZIP

City, State and Zip Code for the responsible person.

Vaccination

Identified vaccine group of the immunization.

Date

Date when the immunization was administered.

Trade Name

Trade Name of the immunization.  If a Trade Name can not be found, this field will be blank.

Organization

Remote organization that owns the immunization.  If an organization is not returned, this field will be blank.
