Public Immunization Record Access |
Purpose |
The Public Immunization Record Access feature gives you the ability to search for and view an immunization record in the Virginia Immunization Registry.
Searching for an Immunization Record |
To search for an immunization record, follow these steps:
Click on the 'Public Immunization Record Access' hyperlink from the login screen for the Virginia Immunization Registry web site. The Public Access Search screen displays.
Enter the first name, last name, and date of birth for the person whose record you are searching for. These are required fields.
You must also enter either the social security number or the Medicaid ID number for the person.
Click on the Search button.
If the system finds one matching record for the data you entered, then the system shows the Public Access Display screen. Please see the Data on the Public Access Display Screen section for explanation regarding this screen.

These are the buttons you can use on the Public Access Search screen:

When you click on the Search button, the system searches the Virginia Immunization Registry for the record of the person whose information you entered on this screen. |

When you click on the Home button, the system takes you back to the home page for the Virginia Immunization Registry web site. |

When you click on the Help button, you will see this help screen, which gives you information on how to use the Public Immunization Record Access feature. |
Reasons the System Cannot Find the Record |
If you try to do an immunization record search but cannot access the record, you may have mistyped some of the person's information. Please double check that you have entered the information correctly on the screen, and perform the search again. Here are some reasons the system may still be unable to find the immunization record you are searching for:
The person's immunization record may not have been recorded in the Virginia Immunization Registry.
The person's immunization record is in the system, but there is no social security number or Medicaid ID stored in the registry.
The person's immunization record is in the system, but the name, date of birth, social security number or Medicaid ID is stored incorrectly.
Possible duplicate records are in the registry because the same social security number or Medicaid ID is stored in different records.

The following table describes the fields you will see on the Public Access Search screen after the person's immunization record is successfully accessed.
Field Name |
Description |
Client Name |
The first, middle, and last name of the person whose immunization record you are viewing. |
Mother's Maiden Name |
The mother's maiden name (first and last name) for the person whose immunization record you are viewing. |
DOB |
The date of birth for the person shown in month/day/year format. |
Gender |
The gender of the person. M stands for male. F stands for female. |
Race |
The race of the person. American Indian or Alaska Native, Asian, Native Hawaiian or Pacific Islander, Black or African-American, White, Hispanic or Latino, or Other Race are the choices. |
Ethnicity |
The ethnicity for the person. |
Comments |
These comments are notes that indicate special considerations that health care workers need to take into account when administering an immunization. |
Reactions |
These indicate that a reaction was recorded on a particular day and may have been associated with a dose of vaccine. |
Vaccine Group |
The name of the vaccine that the person received. |
Date Administered |
The date when the person received the vaccine. |
Series |
This indicates which number in the series the dose of vaccine fulfills. For example, "2 of 3" means that the child needs three doses of this vaccine and this dose is the second dose of the three. |
Vaccines Recommended |
This is a listing of vaccines, showing if the whole vaccine series has been completed or if the person is due for more doses of the vaccine. The legend for the immunization status provides more detail about whether the person needs more of the vaccine and when he or she needs it. |
Buttons on the Public Access Display Screen |
You can use the following buttons on the Public Access Display screen.
