Purpose |
The Edit Client Prophylaxis screen is used to view and update information on or delete a prophylaxis that was previously recorded on a client's prophylaxis history record.

Information Provided |
The Edit Client Prophylaxis screen displays the information held in the application database for the prophylaxis selected from the View Client Prophylaxis screen.
Client Information
The Edit Client Prophylaxis screen displays a header titled Client Information at the top of the page. This header includes client demographics, a scrolling list of client symptoms, and other client-distinct information.
Edit Client Prophylaxis
This form will be displayed when the user clicks on the edit icon from the View Client Prophylaxis screen. The Edit Client Prophylaxis form contains the following information.

Product Description |
This field displays the product description of the selected prophylaxis and is a non-editable field. |
Lot |
This field displays the product lot of the selected prophylaxis and is a non-editable field. |
Quantity |
This field indicates the number of prophylaxis administered. Update this field by selecting a number from the corresponding pick list. A maximum of six prophylaxis can be administered at one time. |
Partial Dose |
This field allows you to specify whether the selected prophylaxis was a "partial" prophylaxis. "Partial" meaning something went wrong when the prophylaxis was being given and was not administered entirely. Check the box to indicate that the dose was a "partial" dose. Click on the Save button to save your changes. |
Date Administered |
This field displays the date which the selected prophylaxis was given to the client and is an editable field. Update this field by typing in a new date in the text box in the format MM/DD/CCYY, or click on the calendar icon to set the date. |
Ordering Authority |
This field names the clinician who had the ordering authority to let the client have the selected prophylaxis and is an editable field. Update this field by selecting a new clinician from the corresponding pick list. |
Administered By |
This field names the clinician who actually gave the selected prophylaxis to the client and is an editable field. Update this field by selecting a new clinician from the corresponding pick list. |
Event |
This field represents the event in which the client was exposed and in need of the prophylaxis. Update this field by selecting a new event from the pick list. |
Entered by Site |
This field denotes the site where the prophylaxis inventory came from and is a non-editable field. |
Input Source of Record |
This field indicates how the prophylaxis was entered into the system. The following descriptions explain the items that display: Created Through User Interface: The prophylaxis was manually entered by someone through the system. Created Through Batch Load: This form of entry may be created in a future release. Currently all prophylaxis will be created through the user interface. |
Reactions |
The reactions field displays a table listing a CDC-specified list of possible reactions for the selected prophylaxis. Previously selected reactions will be denoted by having bold red font, and the check box in the column next to the reaction will be checked. Update the reactions table by checking or un-checking the appropriate check boxes. |

When the Save button is clicked, all updates made to any of the editable information fields will be saved in the database for the selected prophylaxis. After the edited information is saved, you will be returned to the View Client Prophylaxis screen. |

When the Delete button is clicked, you will be prompted to make sure you want to delete the selected prophylaxis. Click the Ok button to continue with the delete or click the Cancel button to stop this process. After the registry deletes the selected prophylaxis from the clients history, you will be returned to the View Client Prophylaxis screen. |

When the Cancel button is clicked, any changes made to the selected prophylaxis will be lost, and you will be returned to the View Client Prophylaxis screen. |
