View Order Screen |
| Purpose |
The View Order screen allows you to view a read-only version of a vaccine order. Note: only orders with a “sent to CDC” or “Complete” status are read-only.

This section contains information on the organization ordering vaccine.

Field Name |
Description |

VFC PIN |
Pin number automatically assigned to each organization on the Site screen. |
|
Site |
Name of the site that is ordering vaccine. |
|
User |
Name of the person ordering vaccine. |
|
Site Phone |
Phone number associated with the ordering organization. |
|
Order Date |
Date the order was created. |
|
Provider Status |
Status of the ordering organization. Only Active organizations can order vaccine. |
|
Direct Ship Allowed |
Indicates if direct ship vaccines appear on the screen to order. |
|
Delivery Contact |
Person to be contacted with questions concerning the order. This person is assigned on the Site screen. |
|
Delivery Address |
Address to which the ordered vaccine will be delivered. |
|
Order ID |
ID number assigned to the order when it was created |
This section contains all historical notes created for this order. Each note will contain the date and time stamp the note was created along with the user name of the creator.
This section is divided into two parts. Pediatric/State Supplied Vaccines intended for pediatric use and Adult/State Supplied Vaccines intended for adult use.

The manufacturer of the vaccine in the corresponding row. |

NDC |
The NDC number of the vaccine in the corresponding row. |

Lot Number |
The lot number of the vaccine in the corresponding row. |

Expiration Date |
The expiration date of the vaccine in the corresponding row. |
